Level 5: Performance & Optimization

Measure what matters. Look for patterns. Use data to ask better questions. Then remember that you are a human being, not a spreadsheet.

I love information. I love understanding why something is happening, looking for patterns and having actual data instead of simply guessing. But I also think we have reached a point where it is possible to track almost everything about ourselves, and more information is not automatically better information.

A watch can tell us our heart rate. A ring can estimate sleep stages. A glucose monitor can show us what happens after a meal. Laboratory testing can give us hundreds of numbers. Our phones can count steps, workouts and screen time.

The question is not whether we can measure something.

The question is: what are we going to do with the information?

That is what this section is about. Data should help us understand our bodies and make better decisions. It should not become another source of fear, perfectionism or pressure.

Start With the Basics

Before ordering an enormous panel of specialty biomarkers, there is a lot of valuable information available through ordinary preventive care and clinically appropriate testing.

What should be checked, and how often, depends on age, sex, health history, medications, symptoms, family history and risk factors. There is no universal 'optimization panel' that every person needs.

Depending on the individual, clinicians may use information such as:

  • Blood pressure. One of the most useful and accessible cardiovascular health measurements.

  • Blood glucose and A1C. Used to evaluate glucose regulation and screen or monitor diabetes when appropriate.

  • Lipid testing. Cholesterol and triglyceride measurements help assess cardiovascular risk; additional lipid-related markers may be useful in selected people.

  • Complete blood count. Can provide information about red cells, white cells and platelets.

  • Metabolic testing. Common panels can provide information related to kidney function, liver-related markers, electrolytes and glucose.

  • Thyroid testing. Appropriate tests depend on symptoms, history and clinical context.

  • Iron status. Ferritin and other iron studies can be useful when iron deficiency or overload is a concern.

  • Vitamin or nutrient testing. Certain nutrients such as vitamin B12 or vitamin D may be appropriate to measure in particular people, but not every nutrient requires routine screening.

  • Hormonal testing. Can be useful when there is a clinical reason, but hormone interpretation depends heavily on age, reproductive stage, medications, timing and the specific hormone being measured.

A Lab Result Is Not a Diagnosis

One of the easiest things to do when we have access to our own results is to see a number highlighted in red and immediately assume something is terribly wrong.

Laboratory reference ranges are tools. They are generally developed from measurements in a reference population and can vary among laboratories, methods and patient groups. A result slightly outside a range does not automatically mean disease, and a result inside a range does not automatically answer every clinical question.

Results have to be interpreted in context: symptoms, trends, medications, hydration, illness, fasting status, time of day and other variables can matter.

This is also why repeating a test can sometimes be more informative than reacting dramatically to one isolated value.

What About 'Normal' Versus 'Optimal'?

This is a phrase you will hear constantly in wellness and functional medicine: 'Your labs are normal, but they are not optimal.'

Sometimes there is a legitimate point behind that conversation. Clinical decisions are not always made by waiting until a value crosses one simple laboratory boundary. Risk can exist on a continuum, and treatment targets for certain conditions may be different from a laboratory's general reference interval.

But 'optimal' is not a magic scientific category that allows someone to invent a narrower range for every biomarker.

If someone tells me my number needs to be pushed toward an 'optimal' target, I want to know: Optimal according to what evidence? For someone with my age, health history and risk factors? Does changing this number improve an outcome that matters? And what are the risks of trying to change it?

Sometimes the answer is compelling. Sometimes the answer is a supplement package.

Trends Can Tell a Better Story Than Snapshots

I am a big believer in looking at patterns.

A single resting heart rate, one blood pressure reading, one night of poor sleep or one unusual lab result is a snapshot. Repeated measurements under reasonably consistent conditions can show whether something is actually changing.

That does not mean every biomarker needs to be checked constantly. Some change quickly. Others change slowly. Repeating tests too frequently can waste money and create noise rather than insight.

The right question is not 'How often can I measure this?' It is 'How often would measuring this reasonably change a decision?'

Wearables: Incredible Tools With Important Limitations

Wearable technology has made personal health data incredibly accessible. Smartwatches, rings and fitness trackers can estimate or measure things such as steps, heart rate, activity, workouts, sleep duration, respiratory rate, blood oxygen on supported devices, temperature trends and heart-rate variability.

I think this can be incredibly useful, especially for trends.

But a consumer wearable is not automatically equivalent to a medical diagnostic device, and accuracy varies by device, metric, activity, skin contact and algorithm.

A wearable can give us clues. It should not diagnose a medical condition simply because an app produced an unusual number.

Heart Rate and Resting Heart Rate

Heart rate is one of the more straightforward measurements many wearables provide, although wrist-based optical sensors can still be affected by movement, device fit and other factors.

Resting heart rate trends can sometimes reflect changes in fitness, illness, recovery, stress, medications and other factors. A persistent or symptomatic change deserves more attention than obsessing over a one-beat difference from yesterday.

Heart-Rate Variability: Useful, but Easy to Overinterpret

Heart-rate variability, or HRV, describes variation in the time between heartbeats. It is influenced by autonomic nervous-system activity and can change with sleep, training, illness, alcohol, stress and many other factors.

The absolute number can differ significantly among people and devices. For most consumers, comparing your own trend using the same device and similar conditions is generally more useful than comparing your HRV with someone else's number online.

A low score one morning is information. It is not a moral judgment about how well you took care of yourself yesterday.

Sleep Tracking: Helpful Estimates, Not a Sleep Laboratory

Wearables can be useful for estimating sleep timing and duration and noticing broad patterns. They are much less reliable at perfectly identifying every sleep stage compared with clinical sleep testing.

If your tracker says you received 47 minutes of deep sleep, that number should not become a diagnosis.

If you have significant daytime sleepiness, chronic insomnia, loud snoring, witnessed breathing pauses or other concerning sleep symptoms, a wearable cannot replace an appropriate medical evaluation or formal sleep testing when indicated.

When Sleep Tracking Starts Hurting Sleep

There is even a term used in sleep medicine literature: orthosomnia. It describes an unhealthy preoccupation with obtaining perfect sleep data that can actually increase anxiety around sleep.

I think this is such an important concept.

Imagine waking up feeling rested, opening an app, seeing a sleep score of 62 and immediately deciding you must feel terrible.

Your device is supposed to provide information about your sleep. It is not supposed to tell you whether you are allowed to feel rested.

If tracking is making you anxious, keeping you awake or causing you to obsess over every score, taking a break from the data may be healthier than collecting more of it.

Continuous Glucose Monitors

Continuous glucose monitors, or CGMs, measure glucose in interstitial fluid throughout the day and night. They are transformative medical tools for many people with diabetes and can reveal patterns that finger-stick measurements may miss.

CGMs have also become popular in the wellness world among people without diabetes.

Seeing how glucose changes after meals, activity, sleep or stress can be interesting, but every glucose rise after eating is not automatically harmful. Glucose is supposed to change after we consume carbohydrate.

For people without diabetes, the clinical value of using a CGM purely for wellness or optimization is still being studied. If someone chooses to use one, I would want the information interpreted in context rather than turning every meal into a pass/fail test.

Blood Pressure: One of the Least Glamorous and Most Useful Things to Track

Blood pressure is not nearly as exciting as a longevity score or a new wearable metric, but it is incredibly important.

Home blood-pressure monitoring can be useful for some people when done correctly. Technique matters: appropriate cuff size, correct positioning, sitting quietly beforehand and taking readings under consistent conditions all affect the result.

This is a perfect example of why better data does not always require more expensive technology.

Body Weight and Body Composition

A scale is another tool that can either provide useful information or completely take over someone's relationship with health.

Body weight naturally fluctuates because of hydration, food intake, bowel contents, hormones and other factors. If weight is something you are intentionally monitoring, trends over time are usually more meaningful than reacting to every daily change.

Body-composition tools can estimate fat mass, lean mass and other measures, but consumer devices vary in accuracy and hydration can influence results. The method matters.

And health is not contained in one number on a scale. Strength, mobility, cardiovascular fitness, metabolic health, function and how you feel all matter too.

Your Body Is Data Too

This may be my favorite part of this entire section.

We have become so good at collecting numbers that sometimes we forget to ask ourselves how we actually feel.

Energy is data. Mood is data. Pain is data. Digestion is data. Strength is data. Concentration is data. Daytime sleepiness is data. Headaches are data. Menstrual changes are data. How quickly you recover is data. How you function during an ordinary day is data.

If your watch says you slept beautifully and you can barely keep your eyes open, the watch does not automatically win.

Objective data and subjective experience are strongest when we use them together.

Symptom Tracking Can Be Surprisingly Powerful

You do not need a $500 wearable to identify patterns.

A simple notes app, journal or calendar can sometimes reveal more useful information than an expensive device.

If you are trying to understand a recurring symptom, consider tracking only the variables that could realistically help answer the question.

  • Date and time.

  • The symptom. What happened and how severe was it?

  • Sleep. Approximate duration and whether it felt restorative.

  • Food or hydration. Only when relevant to the question you are investigating.

  • Medication or supplement changes.

  • Menstrual or hormonal timing. When relevant.

  • Exercise or unusual exertion.

  • Stress or significant events.

  • Anything else that clearly preceded the symptom.

The goal is to find patterns, not document every second of your existence.

Tracking Hormones and Cycles

Hormones deserve context. Menstrual-cycle tracking can help some people identify patterns in symptoms, bleeding, mood, headaches, sleep or energy.

Perimenopause can make cycles and symptoms much less predictable, and hormone levels can fluctuate substantially. A single hormone measurement may not always explain what someone is experiencing.

For people using hormone therapy, appropriate monitoring depends on the therapy, route, indication, medical history and clinician's plan. More frequent testing is not automatically better care.

Nutrition and Hydration Tracking

Food tracking can be useful for answering a specific question: Am I eating enough protein? Am I consistently getting fiber? Am I unintentionally under-eating? How much sodium am I consuming? Am I actually drinking the amount of water I think I am?

But tracking every bite indefinitely is not necessary for everyone and can be counterproductive for some people.

Use tracking as a flashlight, not a prison.

Collect enough information to learn something, make the change, and then decide whether continued tracking is actually useful.

The Personal Health Dashboard

If I were building a personal health dashboard, I would not try to fit every possible biomarker onto it. I would choose a small number of things connected to my actual goals.

  • What am I trying to improve? Energy? Blood pressure? Strength? Sleep? Glucose control? A documented deficiency?

  • What is my baseline? Know where you are starting before deciding something worked.

  • What measurement actually reflects the goal?

  • How often does it reasonably need to be measured?

  • What symptoms should I track alongside it?

  • What change would count as meaningful improvement?

  • What result would make me contact my healthcare provider?

  • What am I going to do differently based on the result?

  • When will I reassess whether tracking is still useful?

Before You Buy Another Health-Tracking Device

  • What does it actually measure? Not simply what the marketing says it 'knows' about your body.

  • Is the metric measured or estimated?

  • Has the device been independently validated for that metric?

  • Is it FDA-cleared for a medical function, or is it a general wellness feature? Those are not the same thing.

  • Will this information change something I do?

  • Can I obtain the information more simply?

  • Will seeing the number motivate me or make me anxious?

  • Can I export or share useful data with a clinician if needed?

  • What happens to my health data? Review privacy practices when that matters to you.

More Testing Is Not Always More Personalized

There is a tendency in optimization medicine to equate more testing with better care.

I do not think those are automatically the same thing.

A test is valuable when it measures something accurately enough to be useful, answers a meaningful question and has an interpretation that can reasonably change care.

A test that produces 200 numbers but leaves you with 40 'abnormalities' nobody knows how to interpret may create more confusion than clarity.

Before paying for a specialty test, ask: Is this test clinically validated for the purpose we are using it for? What does an abnormal result mean? What would we do differently?

If there is no clear answer, sometimes the most optimized decision is not ordering the test.

When Tracking Becomes Too Much

I want this section in here because optimization should improve our lives, not consume them.

Consider stepping back if you notice that you:

  • Check health metrics repeatedly throughout the day.

  • Feel anxious when you cannot wear your device.

  • Change how you believe you feel because an app gives you a poor score.

  • Avoid normal foods or activities because you fear a temporary metric change.

  • Keep adding tests because normal results do not reassure you.

  • Spend more time analyzing your health than living your life.

  • Feel that every number has to be perfect.

Data is supposed to give us perspective. If it starts taking perspective away, that is useful information too.

My Favorite Rule: Track With a Purpose

I do not want data simply because I can collect it.

I want to know what question I am asking.

Maybe the question is: Is this supplement correcting a documented deficiency? Is my blood pressure improving? Am I recovering from workouts better? Am I actually sleeping longer? Is this therapy changing the outcome we hoped it would change?

Then we choose the best reasonable measurement, establish a baseline, make the intervention and look again at an appropriate time.

That turns tracking into a tool instead of a hobby.

What I Want You to Remember

Numbers can help us see things we might otherwise miss.

They can help us catch problems earlier, recognize trends, personalize treatment and have better conversations with healthcare providers.

But numbers need context.

A laboratory range is not your identity. A wearable score is not your worth. A bad night is not a trend. A glucose spike is not a moral failure. And one abnormal result does not tell the entire story.

Measure what matters. Look for patterns. Use data to ask better questions.

Then put the device down and go live your life.
